NO PURCHASE OR SPECIFIC TREATMENT NECESSARY TO ENTER OR WIN. Making a purchase will not increase your chances of winning. The Addressing Psoriasis™ Contest ("Contest") begins at 12:00:00 (Noon) PM Eastern Time ("ET") on Monday, March 2, 2009 and ends at 11:59:59 PM ET on Thursday, April 30, 2009 (“Contest Period”). Void Outside the 50 United States and District of Columbia (“D.C.”) and where prohibited.

  1. ELIGIBILITY: The Contest is open to legal residents of the 50 United States and D.C. who are 18 years of age or older at the time of entry, have a confirmed medical diagnosis of moderate to severe plaque psoriasis, and are able to travel to New York City during the four-day period, as yet to be selected by sponsor, but within the dates of August 31-September 5, 2009, and participate in all activities relating to the Addressing Psoriasis™ fashion show that are described below. By registering, entrants confirm age and diagnosis, and consent to having entry information and medical diagnosis shared with judges. Upon winning, entrants will be required to produce proof of age, proof of diagnosis via a letter from their physician which verifies diagnosis of moderate to severe plaque psoriasis and agree to allow their information provided on entry to become public knowledge. Employees, officers and directors of Amgen Inc. and Wyeth Pharmaceuticals (“Sponsors”), and RealTime Media, Inc. (together with Sponsor, "Contest Entities”), their respective parent companies, affiliates, subsidiaries, advertising, promotional, fulfillment and marketing agencies, and members of the immediate families (parent, child, sibling and spouse) or households (whether related or not) of any of the above are not eligible to participate in the Contest. By entering the Contest, participants agree to release, discharge and hold harmless Contest Entities and American Academy of Dermatology, National Psoriasis Foundation and Psoriasis Cure Now ("Program Participants"), as well as their respective parent companies, affiliates, subsidiaries, retailers, agencies and the respective officers, directors, employees and agents of each of the foregoing, from and against any and all damages whether direct or indirect, which may be due to or arise out of participation in the Contest, or in Contest-related activities, or the acceptance, use/misuse or possession of any prizes that may be awarded to them. By participating, participants agree to abide by and be bound by these Official Rules and the decisions of the Sponsor, as defined in further detail below, which shall be final and binding in all respects. All federal, state and local laws and regulations apply. Program Participants do not endorse or recommend any products or services.
  2. HOW TO ENTER: No purchase or specific prescription necessary to enter or win. LIMIT: Limit one Contest entry per person/per email address for length of the Contest. Subsequent entries from the same person and/or email address will be disqualified. Potential winners who are discovered during prize verification to have entered or attempted to enter more than once may be disqualified at the sole discretion of the judges.

    The five (5) phases of this Contest are as follows:

    1) Phase 1: Call for Entry: The Contest Entry Phase begins at 12:00:00 (Noon) PM Eastern Time ("ET") on Monday, March 2, 2009 and ends at 11:59:59 PM ET on Thursday, April 30, 2009.

    To submit an entry into the Contest, go to the following Web site address: http://www.addresspsoriasis.com (the “Contest Web site”). Entrants will be asked to complete a registration form and must enter their true and correct information in all required fields. Once all required fields are entered, the entrant will click the “Continue” button to complete the registration process. Once the registration form is completed, entrants must submit a video or essay with optional photo to be entered into the Contest. (See “Entry Details” below).

    Entry Details:
    Contestants must submit a video testimonial or a personal essay (with optional photo) explaining his or her inspirational story of how he or she has overcome the challenges of living with moderate to severe plaque psoriasis and how the condition has impacted his or her personal style and the clothing he or she wears. Video must be in .MOV or .WMV format. Video must be 90 seconds or less in length and must not exceed 5 MB in size. Essay must be 300 words or less. The Optional Photo should not exceed 3 MB in size. Should entrants experience any technical difficulties with any aspect of submitting to the Addressing Psoriasis™ contest, they may send an email to addresspsoriasis@promosupport.com. Requests will be answered promptly.

    Once the Video or Essay with Optional Photo entry is submitted, entrants will be officially entered into the Contest. Contest entries cannot be revised or altered once submitted. Once submitted, all submitted components become the property of the Contest Entities and will not be returned. By submitting an entry, you irrevocably grant to Contest Entities, Program Participants their affiliates, agents and licensees the right to unconditionally and perpetually reproduce, copy, store, transmit, publish, display, adapt and/or otherwise use or reuse your name, statements, biographical material, and submission as well as any other materials arising out of your participation in the Contest, including without limitation your personal information, in any manner or media whatsoever (whether known or hereafter devised), worldwide, for advertising and promotional purposes. Subject to applicable law, you waive any rights of privacy, intellectual property rights, and other rights that may preclude Contest Entities from using or disclosing your submission. You release Contest Entities from any legal claims arising out of any use or disclosure of your submission consistent with these Official Rules. All accepted entries will be screened and may be published.

    Information Regarding Submissions:
    The Contest Entities shall have the right to disqualify any entries that they deem to be obscene, vulgar, sexually explicit, lewd, derogatory, inappropriate, or otherwise not in good taste, as determined by the Contest Entities in their sole discretion.

    LICENSE: Entrant grants Contest Entities and their designees the right, but not the obligation to edit, remove, modify, publish, license, print, transmit, display or otherwise use the Video or Essay with Optional Photo entry, the description, and all accompanying personally identifiable information entrant submits (collectively, the “Submission”), via all forms of media now known or hereafter devised (including http://www.addresspsoriasis.com) worldwide, in perpetuity, without notice to entrant and without compensation.

    WARRANTY: Entrant represents and warrants that (i) entrant’s Submission was submitted by entrant, (ii) the Submission is original to entrant and does not infringe upon or otherwise violate any right of any third party, and (iii) entrant has all rights necessary to submit the Submission, including all necessary authorizations and permissions from any persons included in the Submission, (iv) that the Submission will not include unlawful, threatening, libelous, defamatory, obscene, pornographic or other material that would violate the law; or (iv) advertising for third-party products or services.

    Video and Essay with Optional Photo entries submitted in connection with this Contest (a) must be intended for a family audience and contain only content that, in the sole and unfettered discretion of the Sponsor, is suitable for a general audience; (b) must not, in the sole and unfettered discretion of the Sponsor, contain any nudity, sexually explicit, disparaging, libelous or other inappropriate content; (c) must not in the sole discretion of the Sponsor, contain any commercial content that promotes any product or service; (d) must not contain any trademarks, copyrighted works or other intellectual property. Any elements appearing in the Video or Essay with Optional Photo entry, including without limitation stills, clip art, supers, or other materials (collectively, “Elements”) used must be entirely original, created and performed by entrant. Use of any Elements or other materials that are not original to entrant, not in the public domain, or that are not an Authorized Asset may result in disqualification.

    All Submissions may be posted on the publicly accessible Contest Web site during and following the Contest Period. The Contest Entities are not responsible for late, lost, illegible, incomplete, stolen, garbled or misdirected entries, all of which are void.

    2) Phase 2: Contest Judging: The Contest Judging Phase begins at 12:00:00 AM Eastern Time ("ET") on Friday, May 1, 2009 and ends at 11:59:59 PM ET on Friday June 5, 2009.

    Judging criteria is based on the submission of a video testimonial or a personal essay explaining your inspirational story of how you have overcome the challenges of living with moderate to severe plaque psoriasis and how the condition has impacted your personal style and the clothing you wear. A photo may also be submitted along with your essay if you feel it helps tell your story, but appearance will not be considered as part of the judging criteria. Winners will be selected based on the inspirational nature of their story and not on appearance.

    Contestants must meet the following eligibility criteria:

    • Must have a current medical diagnosis of moderate to severe plaque psoriasis.
    • Must be 18 years of age or older.
    • If selected, the winner must be able to travel to New York City during the four-day period, as yet to be selected by sponsor, but within the dates of August 31-September 5, 2009.
    • Winners must agree to participate in all activities pertaining to the Addressing Psoriasis™ Fashion Show, including but not limited to providing measurements for clothing selection and development purposes, participating in fittings for clothing as appropriate, participating in the dress rehearsal the day before the Fashion Show, participating in a hair and makeup styling for the show, modeling clothing at the show, and speaking in a public manner about their inspirational story and participation in the campaign (including, but not limited to, media)
    Entries must meet the following criteria:

    • Video must be in .MOV or .WMV format
    • Video must be 90 seconds or less in length and must not exceed 5 MB in size
    • Essay must be 300 words or less
    • Optional Photo must be less than 3 MB in size
    • Appropriateness of entry in general (i.e. intended for general audience and does not contain nudity, sexually explicit, disparaging, libelous or other inappropriate content)
    • If you are submitting a video entry, it may not contain other people or copyrighted images/music

    The judging criteria are as follows:

    • 50 percent based on the inspirational nature of your personal story
    • 20 percent based on personal story of how you have overcome the challenge of living with moderate to severe plaque psoriasis
    • 20 percent based on how psoriasis has affected your personal style or clothing choice
    • 10 percent based on creative presentation of your story
    * Entries will not be judged on moderate to severe plaque psoriasis treatments.

    All entries will be reviewed and judged based on the contestants’ abilities to meet all contest rules and criteria. The judging process is based on three levels of review. The first level will be judged by Contest Entities and Sponsors to narrow the applicant pool to 30 contestants. A committee of Sponsor employees will then select the top 15 contestants for the final judging. The final selection of the eight winners will be conducted by a judging committee composed of approximately five representatives from Content Entities, Program Participants or their appointed agents.

    In the event of a tie, the Submission which the Judges deems strongest in "Creativity" will be declared the potential Semi-Finalist or Winner. Any further ties will be broken by the Submission which the Judges deem strongest in "Content/Story." The decisions of the Judges are final.

    Any attempt by a person to use multiple email accounts to enter more than once may result in disqualification at the discretion of the Sponsor. Among other required information, entrants must enter a valid United States postal mailing address at the time of entry in order to participate. PO Boxes are not acceptable, and entries containing a PO Box address will be disqualified. Entries will be deemed made by the authorized account holder of the email address submitted at time of entry. In the event of a dispute regarding the identity of any potential winner, the potential winner will be deemed to be the authorized account holder of the email address submitted at time of entry. The authorized account holder is defined as the natural person who is assigned an email address by an Internet access provider, online service provider or other organization that is responsible for assigning email addresses or the domain associated with the submitted email address. A potential winner must provide Sponsor with satisfactory proof that he/she is the authorized account holder of the email address associated with the potentially winning Contest entry. Entries generated by a script, macro or other automated means will be disqualified. Entries that are incomplete, illegible or corrupted are void and will not be accepted. By entering the Contest, entrant recognizes and agrees that the entry information provided by entrant will be made available to Sponsor; use of such information will be governed by Sponsor’s privacy policy available at http://www.addresspsoriasis.com.

    By entering the Contest, entrants grant Sponsor the right to use and publish entrants’ proper name online in connection with the Contest. The use of the entrant’s name online in connection with the Contest does not signify that the entrant is a winner in the Contest. By accepting a prize, each winner (or his/her parent or legal guardian if winner is 18 but deemed a minor in his/her state of residence) consents to the use of winner’s name, address, likeness and/or quotes attributable to winner for advertising and Contest purposes in all media worldwide without limitation or additional notice or compensation except where prohibited.

    If selected, the winner must be able to travel to New York City during the four-day period as yet to be selected by Sponsor, but within the dates of August 31-September 5, 2009. Winners must agree to participate in all activities pertaining to the Addressing Psoriasis™ Fashion Show, including but not limited to providing measurements for clothing selection and development purposes, participating in fittings for clothing as appropriate, participating in the dress rehearsal the day before the Fashion Show, participating in a hair and makeup styling for the show, modeling clothing at the show, posing for photos/appearing in video footage of fashion show, and speaking in a public manner about their participation in the campaign (including, but not limited to, media). If an entrant becomes unable to fulfill these responsibilities, another winner will be chosen. Additionally, upon winning, entrants will be required to produce proof of age, proof of diagnosis via a letter from their physician which verifies diagnosis of moderate to severe plaque psoriasis and agree to allow their information provided on entry to become public knowledge.

    3) Phase 3: Grand Prize Winner Announcement: Upon final verification, the Contest Web site will be updated to showcase the eight winners of the Contest on or about July 1, 2009.

    4) Phase 4: Public Voting: The Public Voting begins at 12:00:00 PM (Noon) Eastern Time ("ET") on Monday, July 6, 2009 and ends at 12:00:00 (Noon) PM ET on Friday, August 21, 2009. LIMIT: Limit one (1) vote per person or email address, per day.

    Once contest winners are selected, the public will have the opportunity to vote on the most inspirational story of the eight Grand Prize Winners. To submit a vote for the most inspirational story, go to the following Web site address http://www.addresspsoriasis.com and follow the voting instructions. Participants will be required to register in order to vote. Once registered, participants will proceed to the Public Voting page and will be able to submit one (1) vote per day for the most inspirational entry.

    The winner whose entry is deemed most inspirational will be named at the Addressing Psoriasis™ Fashion Show. All prize awards and most inspirational story status are subject to verification of eligibility and compliance with the Official Rules.

    In the event of a tie at either Phase 2: The Initial Contest Judging Stage or Phase 4: The Public Voting Stage, the Submission which the Judges deem strongest in "Creativity" will be declared a winner. Any further ties will be broken by the Submission which the Judges deem strongest in "Content/Story". The decisions of the judges are final.

    5) Phase 5: Most Inspirational Winner Announcement: Upon final verification, the winner of the public voting will be announced at the Addressing Psoriasis™ Fashion Show, which will be held within the dates of August 31-September 5, 2009. The Contest Web site will be updated to announce the winner of the public voting Contest on or about September 9, 2009.

  3. PRIZES AND THEIR APPROXIMATE RETAIL VALUE (“ARV”)


    Eight (8) Grand Prize Winners: The Grand Prize winners will receive a 4 day/3 night trip for two (2) to New York City to participate in the Addressing Psoriasis™ Fashion Show, within dates as yet to be selected by the Sponsor but within the timeframe of August 31-September 5, 2009. The trip includes round-trip coach air transportation for the winner and guest, standard hotel accommodations (based on double occupancy) as selected by the Sponsor, round-trip ground transportation to/from the airport and hotel, an outfit designed for winners to wear in fashion show, hair and make up styling, $100 per day for expenses related to the trip, a group dinner, and transportation to/from the hotel and the fashion show venue. Approximate Retail Value (“ARV”): $4,600

    Specifics of all prizes are at the Sponsor’s sole discretion. Total ARV for all Prizes in this Contest is $36,800.

  4. PRIZE AWARDING: Prizes will only be awarded upon winner verification and final approval of Sponsor and its designees. No prize substitution, cash equivalent of prizes, transfer or assignment of prize permitted, except by Sponsor in its sole discretion, or as otherwise provided herein. Sponsor reserves the right to award a prize or portion thereof of equal or greater value if any prize or portion thereof becomes unavailable. Potential Winners will receive notification that he/she has won via electronic mail, phone, and/or mail. Winners may each be required to execute an affidavit of eligibility, liability release, tax acknowledgment form and, except where prohibited by law, a publicity release and return within seven (7) business days of issuance of notification. Noncompliance within this time period or failure of potential winner to respond after seven (7) notification attempts may result in disqualification and, at Sponsor’s discretion; an alternate winner may be selected. Contestants must provide written and signed confirmation that prohibits them from endorsing (including “implied” or “indirect” endorsement that might be inferred or derived by the public) treatment brands or products, or pharmaceutical companies that develop treatments both in their submission and later if they are winners in public. If the winner of any Contest prize cannot be contacted; is ineligible; fails to properly claim the prize in accordance with these Official Rules; if the prize notification/prize is returned undeliverable; or if the winner otherwise fails to fully comply with these Official Rules, he/she will forfeit that prize and an alternate Winner will be chosen from among the remaining eligible entries. Sponsor reserves the right to award only the number of prizes stated in Rule #3.
  5. DISQUALIFICATION: Contest Entities are not responsible for: (i) electronic transmissions or entries that are lost, late, stolen, incomplete, illegible, damaged, garbled, destroyed, or misdirected; (ii) any problems or technical malfunctions, errors, omissions, interruptions, deletions, defects, delays in operation of transmission, communication failures, destruction or unauthorized access to, or alteration of, entries; (iii) failed or unavailable hardware, network, software or telephone transmissions, damage to entrants’ or any person’s computer and/or its contents, or causes beyond Sponsor’s reasonable control that jeopardize the administration, security, fairness, integrity or proper conduct of this Contest; (iv) any entries submitted in a manner that is not expressly allowed under these rules; or (v) incorrect or inaccurate entry information whether caused by entrants or by any of the equipment or programming associated with utilized in the Contest. All incomplete or non-conforming entries will be disqualified. If for any reason this Contest cannot be executed as planned, including but not limited to infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of Sponsor that corrupt or affect the security, administration, fairness, integrity or proper conduct of this Contest, or if this Contest is compromised or becomes technically corrupted in any way, electronically or otherwise, or if fire, flood, explosion, war, embargo, government requirement, fraudulent, criminal or grossly negligent acts, security breaches, civil or military authority, terrorism, acts of God, acts or omissions of carriers, or any other cause beyond Sponsor’s control, interferes with the Contest proper conduct as contemplated by these Official Rules, as determined by Sponsor in its sole discretion, the Sponsor, may, without prior notice, terminate, modify or suspend the Contest, in whole or in part. If this Contest is terminated before the original end date, Sponsor will award the Prizes by random drawing from among all eligible, non-suspect entries received as of the termination date/time. In the event of termination of the Contest, Sponsor will post notice regarding the termination on the Web site. Sponsor reserves the right, at its sole discretion, to disqualify and prosecute any individual who tampers with or attempts to tamper with the entry process or who enters using any automatic or programmed entry duplication method.

    The Contest Entities do not assume any responsibility for any disruption in the Contest caused by circumstance or other unforeseen events outside of the control of the Contest Entities. In the event there is a discrepancy or inconsistency between disclosures and other statements contained in any Contest materials and the terms and conditions of these Official Rules, these Official Rules shall prevail, govern, and control.
  6. TAX INFORMATION: All federal, state, local, and other income taxes on prizes are the sole responsibility of the applicable winner, and each winner may be issued an IRS Form 1099 for the value of his/her prize.
  7. GENERAL INFORMATION: This Contest is conducted in English (which will be given its everyday ordinary meaning), and any and all disputes will be resolved in the appropriate courts in the Commonwealth of Pennsylvania, exclusively, and subject to the laws of the Commonwealth of Pennsylvania. Should there be a conflict between the laws of the Commonwealth of Pennsylvania and any other laws, the conflict will be resolved in favor of the laws of the Commonwealth of Pennsylvania. Entrants irrevocably consent to the personal jurisdiction of said courts and waive any claim of forum non-conveniens or lack of personal jurisdiction they may have.
  8. WINNERS LIST: To receive a list of winners, send a #10 self-addressed stamped envelope for receipt by September 30, 2009 to: The Addressing Psoriasis™ Winners List Request, 1060 First Avenue, Suite 201, King of Prussia, PA 19406.
